  • What do I need to bring from my second visit onwards?
    For your subsequent visits you are requested to bring in: • Your driver license/identification • Your insurance card (required only if there are changes)

  • What is your policy on controlled substance?
    The following medications carry significant risk, can be addictive, and have an abuse potential. In an effort to ensure that our patients are not exposed to unnecessary risk, and receive the best possible care, Magnolia Primary Care LLC has created the following policy for prescribing controlled medications. Amphetamines for Attention Deficit Disorder (ADD) In order for us to prescribe amphetamine medications long term, the patient must have periodic professional psychiatric or psychological evaluation once a year. If such an evaluation has not been done within the past year or if we do not have access to the report, we will assist the patient in scheduling an appointment with a psychiatric or psychological professional. If the patient has been on the medication, as verified by the ALPDMP website, we will continue the medication at the current dose until seen by a psychiatric or psychological professional, at which point we will follow the recommendation of the psychiatric or psychological professional. Opioid Medications for Pain Opioids are appropriate medications to use for acute pain. For significant pain lasting longer than two (2) weeks, specialty referral and/or other methods will be considered. Benzodiazepines We will not prescribe benzodiazepine medications for chronic long-term use. Note: Most of these medications cannot be prescribed for longer than one month and refills are not permitted. Under absolutely no circumstances will prescriptions be written or renewed more often than monthly. We will periodically verify with the Alabama Prescription Drug Monitoring Program (ALPDMP) website to see if the patient is receiving controlled substances from other professionals. If a patient is found to be receiving controlled substances from other professionals and has not disclosed this to Magnolia Primary Care LLC, they will be dismissed from our practice.

  • Which labs do you partner with?
    We do not partner with any specific labs. But, we can fax order request and lab results from most of the local labs.
  • What is an "In Network Lab"?"
    An “In Network Lab” (INL) is a lab that your insurance company prefers you to use. You will get the best prices for yours labs ordered here. Usually an insurance company has more than one INL in the same location. Using an “Out Of Network Lab” can increase the price of lab work significantly. Please contact your insurance company for more details.
